HSES PLAN 2011

In line with the ROUNKO MARINE EXPRESS OIL & GAS SERVICES LTD

HSES policy and International Safety Standards and Guidelines, ROUNKO EXPRESS MARINE OIL & GAS SERVICES LTD (and its operating company's) has designed its 2011 HSES plan to strengthen areas where our performances were high, and also improve on areas where in 2009 we performed below target.

The objective of this plan is to ensure that planning and execution are ingrained in all our activities. Keeping employees adequately informed will be given appraisal to ensure that attitude and techniques are personalized, this outlook sums up our vision for 2011.

• employees and members of the public.
• Minimize the adverse impact on our working environment.
• Work in line with statutory provision.

To achieve the above objectives, ROUNKO EXPRESS MARINE OIL & GAS SERVICES LTD shall:

• Prepare support and be committed to a company HSE policy that will embrace all our employees.
• Continue to provide and maintain a system for supervision and communication of safety policy to all employees.
• Provide adequate training to all employees to enable them carry out their work safely.

Our Strengths were in the following areas:
• Our company did not register any RTA/RTI or injury;
• HSE meetings, safety talks, environmental talks and security talks organized in-house and by our clients were fully attended; and
• Adherence to safety rules.

Cause for concern:
• The company recorded two near misses which could be attributed to the relaxed mood in which the project team found themselves suddenly after successfully completing a field work.
• Five unsafe acts/conditions recorded over the period were attributable to the inability of employees to observe/adhere to safety rules.

HSES Implementation Strategy for 2011

ROUNKO EXPRESS MARINE OIL & GAS SERVICES LTD HSES implementation strategy shall focus on proactive approach to achieve the set targets for the year 2011. This strategy is shown in the table below:

COMMUNITY AFFAIRS HEALTH SAFETY ENVIRONMENT AND SECURITY (HSE – MS) PLAN. 

1.1 HSE-MS MANAGEMENT SYSTEM
1.2 ORGANIZATIONAL STRUCTURE FOR HSE-MS MANAGEMENT

RESPONSIBILITY

1. MANAGING DIRECTOR
The Managing Director shall encourage positive HSE-MS Campaign through the followings:
a. Release of fund for company HSE-MS development
b. Prompt response to all positive HSE-MS recommendations or request etc.
c. Visits to work site at regular intervals
d. Monthly meetings with all heads of departments with view to reviewing and improving HSE-MS policy

2. HSE-MS manager
a. Head of HSE-MS department shall report directly to the Managing Director.
b. Advice on all HSE-MS matters
c. Plan and develop framework for HSE-MS management
d. Administer the execution of company’s employees
e. Represent the company on all HSE-MS matters
f. Liaise with external organization on all HSE-MS matters

3. HSE-MS OFFICER
a. Shall report to the HSE-MS manager
b. Implement all the HSE-MS regulations
c. Arrange supervise and record all site meetings, drill hazards, or other matters relating to HSE-MS.
d. Recommend positive HSE-MS measures etc.
e. Carry out site audits / inspections at regular intervals and report same to the management.
f. Be available on site during work activities to ensure standards are met etc.
g. Liaise with the project Engineer weekly to ensure that procedures are followed.
h. Liaise with the HSE-MS Committee regularly for effective site implementation of HSE-MS regulations.

4. PROJECT SITE SUPERVISOR
Shall be fully responsible for the positive implementation of all site HSE-MS regulations. He shall preside over site HSE-MS meetings and assist the HSE-MS officer on matters affecting the work site. The site supervisor shall be a member of the site HSE-MS committee (For more detail please item HSE-MS manual).

5. SITE NURSE
The site nurse shall among other things report all injury cases promptly to the HSE-MS officer for investigation and remedial measures. Please see more details on the HSE-MS manual.

6. SECURITY OFFICER
Shall report to the HSE-MS Officer. Please see security on the HSE-MS manual

7. FIELD AND / OR OFFICE WORKER
Shall identify and report all hazard to the HSE-MS Officer without delay.

COMPANY CONTINGENCY PLAN MEDICAL EMERGENCY PROCEDURE

1. INTRODUCTION
Given that industrial environment is a potential accident area the need to acquire a workable contingency plan for emergencies become a paramount part of company policy. Consequently ROUNKO EXPRESS MARIE OIL & GAS SERVICES LTD has put in place this emergency procedure which is one line with approval standard.

Every employee must possess a copy of this procedure and must understand its content and significance.

2. IN CASE OF ACCIDENT
1. Do not panic
2. Do not crowd in the scene
3. Inform site engineer immediately
4. Inform safety officer immediately
5. Inform client project engineer immediately
6. Site engineer to commence investigation with his team and makes
recommendation to the management within twenty-four hours.
7. The recommendation must contain among other things the measures for prevention of similar occurrences.
8. A copy of report / recommendations must be sent to client company.

3. IN CASE OF INJURY
1. Do not panic
2. Avoid crowding around victim
3. Do not move victim unless if in danger of additional injury
4. Call the site nurse immediately
5. Inform the site engineer and safety officer immediately
6. Inform client project engineer immediately
7. Alert the retainer clinic immediately
8. Medavac vehicle driver to be informed immediately to get ready for possible
evacuation of victim to retainer clinic.

9. If victim is in critical condition the medevac vehicle MUST head for the nearest clinic or hospital
10. The site nurse must accompany the victim to the hospital
11. Management to be informed within two hours of the injury
12. Investigation of causes and preventive measures to commence.

Having this as a focus ROUNKO EXPRESS MARINE OIL & GAS LTD has put into practice safety measures which are designed to ensure that the well-being of employees, customers / clients and its host community is not endangered by its activities. Consequently, OMS has deliberately included HSE as an integral part of its daily activities and also given the desired commitment.

This handbook (HSE-MS ACTION PLAN) is produced as a guide to achieving an accident free operation and a good company – community relation and respect for the environment in any given project.

POLICY STATEMENT OF ROUNKO EXPRESS MARINE OIL & GAS SERVICES LTD is involved in marine logistics, field sampling services and analysis of soil, and air quality pipeline radiography, supply and installation of an extensive range of oil and gas industry facilities, geographical information system and manpower support development on personal and environmental and safety program.

Its activities include also the supply of materials, instrumentation, electrical, mechanical works and prepare contingency plan.

PROJECT SAFETY GOALS

OMS committee to ensuring that the following safety goals are achieved in all its operations:
• Zero lost time incidents
• Zero recordable injuries
• High level of safety awareness
• Drug and alcohol free project
• Protection of the environment

It is the intent of ROUNKO EXPRESS MARINE OIL & GAS SERVICES LTD that all the work be completed without serious injury.

It is obvious that accidents don’t just happen, but are caused. All possible causes of accident will be avoided.
- OMS will ensure that workers put on appropriate personnel protective equipment (PPE) such as helmet, safety shoes, coverall, eye goggles, hand gloves, etc, while working on site vessels.
- OMS will ensure that correct tools / equipment for the job is made available.
- OMS will ensure that competent / experienced personnel handle the project / job
- Ensure proper safety enlightenment / programmed and training.
- OMS will ensure that workers follow up the safety rules and regulations while working on site / vessels.
- That duly signed work permit is obtained before the commencement of the affected job and follows up specific instructions there in.
- Ensure good house keeping on site / vessels.
- Ensure timely dissemination of safety information
- OMS will make sure workers fully understand the job to be done and the correct procedure and meetings will be organized on site regularly.
In order to maintain a high level of safety awareness on the project site, efforts will be made to inform and encourage the construction crews. This will include items such as:
- Project safety board located around the site

- Safety information flyers posted throughout the site / vessels and
- Fire drills conducted regularly.

Examples of some of the safety flyers are attached.

Drugs: All intoxicating beverages will not be allowed within the company’s premises and at the company’s operations site / vessels.

Smoking: This will not be permitted in clearly designated ‘No Smoking’ areas within the company’s premises and area of operations including vessels.

ACCIDENTS AND NEAR MISS

Classification, Causes & Prevention
An Accident is an unplanned, sudden unforeseen event, which interrupt the normal course of production activity. This may not lead to injury or properly damage.

CLASSIFICATION OF INCIDENTS

Incidents on site / vessels are classified into any of these categories.

Near miss: This accident that narrowly misses its target. No victim is involved. Near miss accident is an indication that something is wrong and could result in subsequent serious accident.
First Aid: Is considered one time treatment of an injury and does not require surgery, sutures, or treatment that requires follow-up care.

Examples include:
• An abrasion, cut or laceration requiring a bandage
• Treatment of only first degree burn
• Use of antiseptic on first visit.

Medical (recordable): If there is work related illness (long term hearing loss, respiratory illness, chemical sensitivity etc) and work related death.
• It also involves medical treatment (other than first aid)
• Loss of consciousness
• Any restriction of work or transfer to other job as a result of injury.

LOST TIME: Lost time wound start if the individual did not report to work at the nest scheduled work shift. Note that lost time would not include the balance of the day in which the individual was injured or scheduled off days.

CAUSES OF ACCIDENT

Accidents are caused, they don’t just happen. The causative factors fall into two broad groups, namely:
1. UNSAFE ACT
2. UNSAFE CONDITION

UNSAFE ACT: Is an act performed by some one without due regard for this personal safety and that of others. (unfortunately) e.g. spilling oil on the floor. Unsafe acts might be a violation of a commonly accepted safe practice or procedure. Unsafe acts are performed due to imperfection. Taking short cut, apathy, in-discipline, age and physical fitness, emotion, lack of proper supervision etc.

UNSAFE CONDITION: This is a situation which is capable of causing an accident e.g. the slipper floor, dimly light workshop etc. Unsafe act can lead to unsafe condition that result in accident.

Accident creates insecurity in work sites, causes loss of production, causes death, pains, injuries, suffering and hardship for individuals, reduces workers morale and even the confidence of the community within which the affected company operates. Other negative impetus include: cost of investigation, loss of employees, cost of recruitment and training of employees, effect on dependants and company, social and economic losses to the

workers and nation, loss of reputation etc. Consequently, all efforts should channeled towards preventing accidents.

ROUNKO EXPRESS MARINE OIL & GAS SERVICES LTD ACCIDENT PREVENTION TECHNIQUES.
Knowing the hazards can prevent all accidents and by ensuring that appropriate safety measures / programmed are put in place. It is obvious that for a given task to be performed the components of functional system which are people tool (Equipment and materials) and environment must act and interact constantly with each other, either directly or indirectly. Accident occurs when there are deficiencies in any of these components. This knowledge forms the basis of our accident prevention techniques.

Prevention by elimination or reducing the sources of potential risks / hazards can be achieved in a numbers of ways:
To minimize / eliminate human errors, a careful selection of workers and adequate professional and safety training before, during and after work is conducted, as a result of which workers will always choose the safe option consciously or unconsciously.

To integrate into the operation, OMS management has:
a. Established definite written safety standard
b. Clearly outlined each employee and supervisor’s responsibility
c. Hold supervisor responsible for those working under him
d. Plan safety into each job process and provide suitable facilities
e. Supervise according to safety method with no deviation
f. Have a sincerity of purpose and insist on safe performance through the safety officer and supervisor to the men on the job.
g. Make frequent inspections on all departments / vessels of condition and practices.

To tackles any deficiency that may emanate from tools, OMS ensures that plants and equipment do not fail and cause injury and damage. This is done through a provision requiring that equipment such as hoist and lifts, chains, rope and lifting tackles, crane and pressure vessel including vehicles and other machines which could fail due to

deterioration from usage or age are tested at prescribed interval of time. The test will determine whether the equipment should continue to be used at its rated capacity down graded or removed from service.

It ensures that the working environment is appropriate, building must have sufficient structural stability, good lighting and proper ventilation.

SAFETY MEETING AND SAFETY AUDIT

SAFETY MEETING: Regularly scheduled safety meetings hold by OMS shall include the following during the first week of each month, a general safety meeting or ALL workers will be organized, thereafter each department / trade will have theirs in turn as follows:
1. Marine operation activities 1st week of every month
2. Operation / drivers 2nd week of every month
3. Mechanical 3rd week of every month
4. Civil 4th week of every month

At least one client safety / project representative will attend the safety meeting.

SAFETY AUDIT
The purpose of the Safety Audit is to review the work practices, site conditions / vessels and anticipate safety concerns for upcoming work and to spot and correct unsafe acts or conditions. The Audit will not be limited to just the construction activities, but will also inspect site facilities as such living quarters, vessels, mess hall and clinic.

The audit provides visible indication of management’s concern for the safety and well being of the workers. Formal audits of the construction site / vessels will be held every week. Those in attendance will include:
• OMS project manager / site manager
• OMS marine superintendent
• OMS safety officer
• OMS marine supervisors
• Clients site manager
• Client’s safety engineering / safety officer
Audit will be assisted with the use of a checklist. Each audit will be documented, indicating the items for correction, area of good practice and plan to avoid anticipated safety problems expected work.
Item for correction shall be addressed immediately and follow up at the next audit.
